What You Need to Know Before You Apply

What is the purpose of this trial?

This trial tests whether the medications doxepin and ramelteon can improve sleep quality for those recovering from total knee replacement surgery. Poor sleep during recovery can worsen pain and slow healing. Participants will receive either doxepin alone, doxepin combined with ramelteon, or a placebo to determine which offers the best results. Individuals who have recently undergone knee replacement and are not currently using certain mental health medications or sleep aids might be suitable for this study. As a Phase 4 trial, this research involves FDA-approved treatments and aims to understand how they can benefit more patients.

Will I have to stop taking my current medications?

The trial requires that you stop taking certain medications, such as SSRIs, SNRIs, MAOIs, prescription sleep aids, and pre-operative narcotics. If you are on these medications, you may not be eligible to participate.

What is the safety track record for these treatments?

Research has shown that both doxepin and ramelteon are generally safe for improving sleep. Studies have found that doses of 1 mg, 3 mg, and 6 mg of doxepin are well-tolerated by adults with insomnia, improving sleep without major side effects. However, some rare sleep-related behaviors, such as sleepwalking, have been reported.

The combination of doxepin with ramelteon is also considered safe. Ramelteon, approved by the FDA for sleep problems, usually does not cause serious side effects. Are You a Good Fit for This Trial?

This trial is for adults aged 18–80 who are having their first total knee replacement surgery. People can't join if they have certain heart problems, use specific antidepressants or prescription sleep aids, have major mental health issues, drink heavily, use narcotics regularly before surgery, have serious sleep disorders or glaucoma, or are pregnant/breastfeeding.

Inclusion Criteria

I am between 18 and 80 years old.
I am having my first total knee replacement surgery.

Exclusion Criteria

Pregnant, breastfeeding, or planning pregnancy within 3 months post-surgery; no pregnancy testing performed, based on self-report
Self-reported use of alcohol in excess of >2 drinks/day (men) and >1 drink/day (women)
I have severe trouble emptying my bladder.
