What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests whether cannabidiol (CBD), a compound from the cannabis plant, can reduce early warning signs of psychosis in people at high risk. It also evaluates the safety of CBD for these individuals. Participants will receive either CBD or a placebo (a pill with no active ingredients) to assess differences in symptoms and safety. Individuals who have experienced warning signs of psychosis, such as unusual thoughts or perceptions, may be suitable for the study. As a Phase 3 trial, this study represents the final step before FDA approval, offering participants an opportunity to contribute to potentially groundbreaking treatment advancements.
Do I need to stop my current medications to join the trial?
The trial requires that you stop taking certain medications, such as antipsychotics at specific doses, valproate, and clobazam, before participating. If you're on these medications, you may need to adjust or stop them under your doctor's guidance before joining the trial.
Is there any evidence suggesting that cannabidiol is likely to be safe for humans?
Research has shown that CBD is generally safe for early psychosis. Studies have found that CBD can reduce symptoms in people with schizophrenia. Doses between 600 and 1000 mg have produced positive results without major safety issues. Short-term use of CBD has also helped those at high risk of developing psychosis. Importantly, these studies reported that CBD was well-tolerated, with few serious side effects.

Are You a Good Fit for This Trial?
This trial is for people aged 12 to 35 who are at high risk of developing psychosis, based on specific early warning symptoms. Participants must not be in another treatment study and, if able to have children, must use effective birth control during the trial.Inclusion Criteria

Screening
Participants are screened for eligibility to participate in the trial
Treatment
Participants are randomised to treatment with oral CBD 300mg twice daily, or a matching placebo, for 104 weeks. Participants will provide various biological samples and complete neuroimaging assessments.
Taper
Participants undergo a 2-week tapering period after the main treatment phase.
Follow-up
Participants are monitored for safety and effectiveness after treatment, with assessments including clinical and biomarker evaluations.

The study tests whether taking cannabidiol (CBD) twice daily by mouth can reduce early signs of psychosis compared to a placebo. It is a randomized, double-blind trial where neither participants nor researchers know who gets CBD or placebo.

Participants will take the intervention for 106 weeks (104 weeks plus a 2-week taper). Doses depend on age and weight: Adults (16 years and older) over 50kg: daily dose 600mg (6ml); 300mg (3ml) twice a day (b.i.d); Adults (16 years and older) less than 50kg: daily dose 10mg/kg; 5mg/kg twice a day (b.i.d); Children (\<16 years old): daily dose 5mg/kg twice a day (b.i.d), following on from a two-week rapid titration schedule. n=188
A healthy control in the context of this trial is someone who does not meet CHR-P criteria or have a diagnosis of a mental health condition. They will not take the trial intervention and attend one trial visit for clinical assessments and biomarker sampling only. n=150
