Inclusion Criteria

I am the parent of a child (10-17) evaluated for suicidal thoughts in the ED and discharged with a safety plan.

Exclusion Criteria

I am proficient in English or Spanish.
Parents/guardians will be excluded if the patient is being admitted or transferred for medical or psychiatric hospital admission
Parents/guardians will be excluded if they are not willing to enroll in MyChart through the EMR
