Implementation Support for Social Determinants of Health

(DEDICATE Trial)

This trial aims to enhance how health centers use electronic health records (EHRs) to connect patients with community resources for non-medical needs, such as housing or food assistance. The research will test whether supporting care teams in using these EHR tools, known as "Implementation Support," increases screenings and referrals to community services. Health centers already using EHRs to manage non-medical needs for at least ten patients may be well-suited for this study. As an unphased trial, this study provides a unique opportunity to contribute to innovative solutions that improve patient care and community support.
