Pharmacist-Assisted Therapy for Heart Failure

(PATRIOT-HF Trial)

This trial aims to determine if a virtual collaboration between pharmacists and cardiologists can improve medication prescriptions for Veterans with heart failure. The goal is to ensure Veterans receive the appropriate heart medications, which can extend their lives, reduce hospital visits, and improve overall well-being. Veterans hospitalized with heart failure in the last three months who are not on optimal heart medications might be suitable candidates for this study. As an unphased trial, it offers Veterans the chance to contribute to innovative care strategies that could enhance their treatment experience.

A previous study showed that virtual consultations with pharmacists and cardiologists are generally safe for patients. During these digital consultations, healthcare professionals review patient records and provide treatment advice. This method focuses on improving the prescription of current medications, so no new drugs are tested, which typically results in fewer safety concerns. Additionally, the virtual consultation itself poses little risk to patients, as it aims to enhance the care Veterans already receive.¹²³⁴⁵

Researchers are excited about the Pharmacist/Cardiologist Virtual Team Digital Consultation because it brings a fresh approach to managing heart failure. This trial will compare the effectiveness of a Pharmacist/Cardiologist Virtual Team with usual care for heart failure patients. Specifically, one study found that this approach improved the use of key heart medications compared to care without pharmacists. Another study demonstrated that patients with heart failure benefited from this method, proving it safe and effective.
