This trial is for adults with heart failure and a low ejection fraction (35% or less) who have a HeartLogic-capable ICD or CRT-D device. Participants must have had recent worsening of heart failure, be able to take their medications as prescribed, and not be pregnant or on dialysis.Inclusion Criteria

Trial Overview
The study compares two ways of using the HeartLogic diagnostic tool in people with heart failure: one group will have changes made based on alert thresholds from the tool, while the other group will keep current settings. Both groups continue standard care.

Heart Logic threshold set to 10 (from 16) will trigger a treatment algorithm, which includes up titration of GDMT
Heart Logic transmission of 16 or greater may be reported to the appropriate device clinic staff by Latitude transmission and then referred to the appropriate treating provider. Medication changes will be based upon physician preference/standards. Repeat transmissions, follow-up, laboratory studies will be per physician preference.
