Neurocognitive Ankle Training for Chronic Ankle Instability
(NATION Trial)
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests a new ankle rehab program called NATION (Neurocognitive Ankle Training for Instability to Optimize Neuromusculoskeletal outcomes) for physically active individuals with ongoing ankle instability. The researchers aim to determine if exercises incorporating mental tasks can improve health, function, and reduce injuries. Participants are divided into three groups: one continues their normal routine, another tries the NATION program, and the third tries a similar program without mental tasks. This trial suits individuals who have previously sprained an ankle, experience frequent ankle instability, and regularly engage in physical activity. As an unphased trial, it offers a unique opportunity to explore innovative rehab methods that could enhance recovery and performance.
Do I have to stop taking my current medications for the trial?
The trial information does not specify whether you need to stop taking your current medications. It's best to discuss this with the trial coordinators or your doctor.
What prior data suggests that this rehabilitation protocol is safe for individuals with chronic ankle instability?
Research has shown that neurocognitive ankle training, such as the NATION protocol, is generally manageable for participants. Studies indicate that incorporating mental tasks into physical therapy can enhance balance and reduce the risk of re-injury for individuals with persistent ankle issues. Although detailed safety information remains limited, these exercises are typically safe due to their focus on balance and coordination. No major reports of adverse effects have emerged, suggesting this method is safe for most individuals. It remains important to follow any specific guidelines provided by healthcare professionals during the trial.12345
Why are researchers excited about this trial?
Researchers are excited about the Neurocognitive Ankle Training (NATION) for chronic ankle instability because it adds a novel twist to traditional rehab methods. Unlike standard physical therapy, which typically focuses on physical exercises to improve strength and balance, NATION integrates neurocognitive exercises that engage the brain, potentially enhancing neuromusculoskeletal outcomes more effectively. This approach might offer a more comprehensive recovery by targeting both the mind and body, aiming to improve not just the physical aspects of ankle stability but also the cognitive processes that contribute to movement and balance.
What evidence suggests that this trial's treatments could be effective for chronic ankle instability?
Research has shown that brain training exercises can help with chronic ankle instability (CAI). Studies have found that this type of training reduces pain and improves balance and ankle function. For example, participants reported less anxiety about injuries and better control over their movements. Another study discovered that adding brain tasks to ankle rehab enhances awareness of body position and overall stability. This trial will test the NATION protocol, which includes brain exercises, for its effectiveness in treating CAI by improving ankle function and lowering the risk of future injuries.16789

Are You a Good Fit for This Trial?
This trial is for physically active people who have chronic ankle instability, meaning they've had at least two episodes of their ankle 'giving way' in the past six months and a history of ankle sprains. Participants must be able to use email/internet and provide consent.Inclusion Criteria
Timeline for a Trial Participant
Screening
Participants are screened for eligibility to participate in the trial
Treatment
Participants undergo a 6-week rehabilitation protocol with neurocognitive exercises, including 18 unsupervised home sessions and 12 supervised sessions.
Follow-up
Participants are monitored for clinical, health, and service member outcomes at 6-weeks and 6-months post-enrollment.
Long-term Follow-up
Participants' medical records are surveilled for surgical interventions related to CAI up to 60-months post-enrollment.

Trial Overview
The study tests a new rehab program called NATION, which combines physical exercises with brain-training tasks to improve ankle stability and overall leg health in people with chronic ankle instability.
How Is the Trial Designed?

Experimental Treatment
Active Control
NATION is a 6-week evidence-based CAI rehabilitation protocol that incorporates neurocognitive exercises. Participants will complete 18 unsupervised home sessions and 12 supervised sessions over the course of 6-weeks.
Participants assigned to the Control arm will be instructed to continue their normal routine.
The Evidence-Based protocol will be identical to the intervention (i.e., NATION protocol), but the exercises will exclude the neurocognitive tasks. Participants will complete 18 unsupervised home sessions and 12 supervised sessions over the course of 6-weeks.
