Amnioinfusion for Neonatal Brain Injury

This trial explores whether amnioinfusion, a procedure that introduces fluid into the amniotic sac, can protect a baby's brain during labor when the mother has chorioamnionitis, an infection in the uterus. The trial targets hypoxic-ischemic encephalopathy (HIE), a condition that arises when a baby's brain receives insufficient oxygen or blood flow, leading to serious health issues. Participants will receive either amnioinfusion or standard care to determine if amnioinfusion reduces stress on the baby's brain. Pregnant individuals in labor with a single baby and diagnosed with chorioamnionitis may be suitable for this study. After birth, researchers will take a small blood sample from the umbilical cord to check for signs of brain stress or injury. As an unphased trial, this study provides a unique opportunity to contribute to important research that could improve outcomes for future pregnancies.

The trial protocol does not specify whether you need to stop taking your current medications. However, all patients will continue to receive normal treatment for infection, so you may be able to stay on your current medications.

Research shows that amnioinfusion is a safe and well-tolerated procedure during labor. It involves adding fluid to the uterus, similar to the natural fluid around the baby, and is often used to reduce stress on the baby during birth.

Studies have found that amnioinfusion can lower the need for cesarean sections due to baby distress. For instance, one study found it reduced cesarean rates by about 30%. Another study showed a decrease in signs of baby distress from 56.7% in those who didn't receive amnioinfusion to 32% in those who did.

Researchers are excited about amnioinfusion for neonatal brain injury because it offers a novel approach to potentially protect the brain before birth. Unlike standard obstetric care, which primarily focuses on managing labor and delivery, this technique involves infusing a cooled solution directly into the amniotic cavity. The use of room temperature lactated Ringer's solution aims to stabilize the environment around the baby, which may help prevent brain injury by maintaining a consistent and supportive intrauterine environment during labor. This technique could be a game-changer in reducing the risk of brain injury in newborns, providing a proactive rather than reactive approach to care.
