This trial is for caregivers of hospitalized veterans with Alzheimer's Disease and Related Dementia who are transitioning from skilled nursing facilities to home. Caregivers must be willing to engage with a web-based program or review health education materials, participate in phone calls, and complete questionnaires.

Inclusion Criteria

Provide care primarily due to a personal relationship, rather than a financial relationship
I use a computer or tablet to go online at least three times a week.

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial
