This trial aims to determine whether certain types of dressings cause allergic reactions after open shoulder surgery. It compares two dressings: one made from silk fibroin and another using 2-octyl cyanoacrylate, a type of medical adhesive. The study will assess the frequency of allergic reactions, the comfort level of each dressing, and the need for additional clinic visits due to complications. Patients who have undergone open shoulder surgery with Dr. James Gregory or Dr. Robert Fullick and have no history of adhesive allergies may be suitable candidates for this trial. As an unphased trial, it offers a unique opportunity to contribute to medical research without the constraints of traditional trial phases.

Is there any evidence suggesting that this trial's treatments are likely to be safe?

Previous studies have shown that silk fibroin presents very few problems with wound healing. For instance, among many cases reviewed, only one person experienced an allergic skin rash, and a few had minor wound issues. Overall, silk fibroin is considered safe and well-tolerated.

Research on 2-octyl cyanoacrylate has found a small number of allergic reactions. Approximately 2.7% of patients experienced an allergic skin reaction, which can cause redness, itching, or a rash. However, these reactions are rare, and most people do not encounter any problems.

What evidence suggests that this trial's treatments could be effective for contact dermatitis?

Research has shown that silk fibroin, one of the treatments tested in this trial, reduces skin allergies more effectively than cyanoacrylate mesh, the other treatment under study. One study found that using silk fibroin to close wounds in joint surgery patients completely prevented allergic skin reactions. Another study demonstrated that silk fibroin significantly reduces skin irritation while still effectively supporting wound healing. Thus, silk fibroin is less likely to cause skin allergies and is a promising choice for wound dressings.678910

Are You a Good Fit for This Trial?

This trial is for people having open shoulder surgery with Dr. James Gregory or Dr. Robert Fullick, who do not have a history of adhesive allergies, wound problems at the surgical site, or use negative pressure dressings.

Inclusion Criteria

I am having open shoulder surgery with Dr. Gregory or Dr. Fullick.

Exclusion Criteria

I have a wound problem at the site where surgery is planned.
Prior documented medical adhesive contact allergy
Negative pressure wound dressings

Timeline for a Trial Participant

Screening

Participants are screened for eligibility to participate in the trial

2-4 weeks

Treatment

Participants undergo open shoulder surgery and receive either silk fibroin or 2-octyl cyanoacrylate mesh dressings

10-14 days
1 visit (in-person) for surgery and dressing application

Follow-up

Participants are monitored for allergic contact dermatitis and other complications related to dressings

6 weeks
Multiple visits (in-person and virtual) for monitoring and assessment
