Liposomal Bupivacaine for Preventing Atrial Fibrillation
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial explores whether administering liposomal bupivacaine, a long-lasting pain relief medication, before heart surgery can prevent atrial fibrillation, an irregular heartbeat, after the operation. Participants will receive either this medication or a saline placebo, a harmless solution, before surgery. Researchers will monitor heart rhythms using a wearable ECG device. Individuals scheduled for coronary artery bypass surgery, who might require additional heart procedures, could be suitable candidates. As a Phase 4 trial, this research involves an FDA-approved treatment and aims to understand its benefits for a broader patient population.
Will I have to stop taking my current medications?
The trial does not specify if you need to stop taking your current medications, but you cannot participate if you are using anti-arrhythmic drugs (except beta-blockers).
What is the safety track record for stellate ganglion blockade with liposomal bupivacaine?
Research has shown that liposomal bupivacaine, used in a procedure called stellate ganglion block, is generally well-tolerated. Studies have found that this nerve block procedure aids recovery after surgery without causing significant side effects. For instance, research on its use for heart rhythm issues did not identify major safety concerns, suggesting it might safely prevent irregular heartbeats post-surgery. Additionally, the FDA has already approved liposomal bupivacaine for other uses, supporting its safety profile. However, as with any medical treatment, individual experiences can vary.12345
Why are researchers enthusiastic about this study treatment?
Unlike standard treatments for preventing atrial fibrillation, which often include medications like beta-blockers or anticoagulants, the use of liposomal bupivacaine offers a novel approach. Researchers are excited about this treatment because it uses a stellate ganglion blockade, which targets nerve activity directly, potentially reducing the risk of atrial fibrillation in a more targeted manner. The liposomal formulation allows for a slow, sustained release of bupivacaine, which might prolong its effects and offer better management of the condition. This method could offer a new way to prevent atrial fibrillation with potentially fewer side effects compared to traditional systemic medications.
What evidence suggests that stellate ganglion blockade with liposomal bupivacaine might be an effective treatment for preventing atrial fibrillation?
This trial will compare the effectiveness of a stellate ganglion block using liposomal bupivacaine with a sham block using a saline placebo. Research has shown that stellate ganglion blockade with liposomal bupivacaine might help reduce irregular heartbeats, including atrial fibrillation. Some studies suggest it can lower abnormal heart rhythms and possibly decrease the risk of atrial fibrillation. Although one study found it did not specifically reduce atrial fibrillation after surgery, it did show improved recovery post-surgery. Additionally, this treatment has effectively controlled other heart issues, such as ventricular tachycardia, suggesting it might help manage atrial fibrillation.12567

Are You a Good Fit for This Trial?
This trial is for adults aged 40 or older who are scheduled to have coronary artery bypass surgery, with or without certain additional heart procedures. People having other types of major surgeries at the same time may not be eligible.Inclusion Criteria
Timeline for a Trial Participant
Screening
Participants are screened for eligibility to participate in the trial
Pre-surgical Treatment
Participants receive pre-surgical stellate ganglion blockade with liposomal bupivacaine or a saline placebo
Postoperative Monitoring
Participants are monitored for postoperative atrial fibrillation using continuous ECG monitoring
Follow-up
Participants are monitored for safety and effectiveness after treatment

Trial Overview
The study compares a nerve block using liposomal bupivacaine before heart bypass surgery to a placebo (saline) injection, aiming to see if it helps prevent irregular heartbeat (atrial fibrillation) after surgery.
