Nerandomilast for Interstitial Lung Disease
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests a new medicine called nerandomilast to determine if it can help people with fibrosing interstitial lung disease (ILD), a condition that can cause lung scarring. The goal is to see if nerandomilast can slow the progression of this disease. Participants will take either nerandomilast or a placebo (a pill with no active medicine) for over two years while doctors closely monitor their lung health. The trial seeks individuals diagnosed with fibrosing ILD within the last three years. As a Phase 3 trial, this study represents the final step before FDA approval, offering participants a chance to contribute to potentially groundbreaking treatment.
Do I have to stop taking my current medications for the trial?
The trial does not specify if you need to stop taking your current medications. However, if you are on certain immunosuppressive or immunomodulatory medications, you need to have been on a stable dose for at least 12 weeks before starting the trial.
Is there any evidence suggesting that nerandomilast is likely to be safe for humans?
Research has shown that nerandomilast is generally well-tolerated by those who take it. Studies have found its safety and tolerability to be consistent across different trials, with few participants discontinuing use due to side effects. Additionally, nerandomilast is approved for treating idiopathic pulmonary fibrosis (IPF), indicating a known and acceptable safety profile for humans. While any medicine can have side effects, evidence from past studies suggests that nerandomilast is a safe option for many people.12345
Why do researchers think this study treatment might be promising for interstitial lung disease?
Nerandomilast is unique because it offers a novel approach to treating interstitial lung disease by specifically targeting inflammation pathways. Unlike standard treatments such as corticosteroids or immunosuppressants that broadly suppress the immune system, Nerandomilast works by inhibiting a particular enzyme involved in inflammatory responses, potentially reducing side effects associated with generalized immune suppression. Researchers are excited about Nerandomilast because its targeted mechanism could provide more effective management of symptoms with fewer adverse effects.
What evidence suggests that nerandomilast might be an effective treatment for fibrosing interstitial lung disease?
Studies have shown that nerandomilast can slow the worsening of lung function in people with pulmonary fibrosis. One study found that it significantly slowed the decline in lung function for those with this condition. Additionally, a review of multiple studies revealed a 59% reduction in the risk of death for patients taking nerandomilast. Importantly, these benefits do not come with extra safety risks compared to a placebo. In this trial, participants will join either the nerandomilast arm or a placebo arm. Overall, nerandomilast appears promising in managing fibrosing interstitial lung disease by maintaining better lung function and potentially increasing survival.13467
Are You a Good Fit for This Trial?
This trial is for adults (18+) diagnosed within the last 3 years with fibrosing interstitial lung disease (ILD), except idiopathic pulmonary fibrosis. Participants must have at least moderate lung function and signs of fibrosis on a lung scan. Some stable immune-suppressing medications are allowed.Inclusion Criteria

Screening
Participants are screened for eligibility to participate in the trial
Treatment
Participants receive either nerandomilast or placebo tablets for up to 2 years and 4 months. Regular visits include lung function tests and HRCT scans.
Follow-up
Participants are monitored for safety and effectiveness after treatment
What Are the Treatments Tested in This Trial?

Trial Overview
The study tests whether nerandomilast tablets can slow down worsening of fibrosing ILD compared to placebo tablets, which look the same but contain no medicine. Participants are randomly assigned to one group or the other and followed for about 2 years.
