Zanubrutinib + Obinutuzumab + Sonrotoclax for Mantle Cell Lymphoma
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests a new combination of three drugs—zanubrutinib, obinutuzumab, and sonrotoclax—to determine their effectiveness in treating mantle cell lymphoma (MCL), a type of blood cancer. The goal is to assess whether this combination effectively controls the disease. Suitable candidates for this trial include individuals diagnosed with MCL who face challenges with standard treatments due to other health conditions, such as heart or lung problems. As a Phase 2 trial, the research focuses on measuring the treatment's effectiveness in an initial, smaller group of participants.
Is there any evidence suggesting that this trial's treatments are likely to be safe?
Previous studies have found the combination of zanubrutinib, obinutuzumab, and sonrotoclax manageable for patients with chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL). Most side effects were mild to moderate, with diarrhea occurring in about 37% of patients.
Zanubrutinib, in particular, has demonstrated a good safety record for patients new to mantle cell lymphoma (MCL) treatment, causing fewer heart-related side effects compared to similar treatments. Although some serious side effects occurred, they were less common.
These findings suggest the treatment could be safe for people with MCL. However, discussing any concerns with a healthcare provider before joining a trial is important.12345Why are researchers excited about this trial's treatments?
Researchers are excited about the combination of Zanubrutinib, Obinutuzumab, and Sonrotoclax for treating Mantle Cell Lymphoma because it offers a novel approach compared to existing treatments. While traditional therapies often rely on chemotherapy, this treatment harnesses the power of targeted therapies. Zanubrutinib is a Bruton’s tyrosine kinase (BTK) inhibitor that blocks signals that help cancer cells survive and grow. Obinutuzumab is a monoclonal antibody that targets CD20 on B-cells, enhancing the body's immune response against cancer cells. Sonrotoclax is designed to inhibit a protein that helps cancer cells evade death, promoting their destruction. This multi-target strategy has the potential to be more effective and have fewer side effects than conventional chemotherapy.
What evidence suggests that this trial's treatments could be effective for mantle cell lymphoma?
Research has shown that the combination of zanubrutinib, obinutuzumab, and sonrotoclax, which participants in this trial will receive, may be promising for treating mantle cell lymphoma (MCL). In earlier studies, many patients who received this treatment had a strong response, with 60% achieving a complete response, meaning no detectable cancer in their blood. Additionally, many patients experienced deep remission, indicating a significant impact of the treatment. This combination also proved effective in other conditions like chronic lymphocytic leukemia (CLL), where it led to quick and deep remissions. The success in these areas suggests potential effectiveness for MCL as well.35678

Are You a Good Fit for This Trial?
This trial is for adults diagnosed with mantle cell lymphoma (MCL). Participants should be healthy enough to undergo treatment and meet specific medical requirements set by the study team. People with certain other health conditions or who can't safely receive these medicines may not qualify.Inclusion Criteria

Screening
Participants are screened for eligibility to participate in the trial
Treatment
Participants receive zanubrutinib, obinutuzumab, and sonrotoclax. Zanubrutinib is administered orally twice daily in each 28-day cycle. Obinutuzumab is administered on days 1, 8, and 15 of cycle 1 and on day 1 of cycles 2-8. Sonrotoclax is initiated on cycle 2, day 1 using a ramp-up schedule.
Follow-up
Participants are monitored for safety and effectiveness after treatment, including evaluation of tumor lysis syndrome and PET CR-uMRD6 rate.
