What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests whether upadacitinib, a Janus kinase inhibitor, combined with topical corticosteroids, can better manage Eosinophilic Esophagitis (EoE), a condition causing symptoms like trouble swallowing and heartburn. Participants will take either upadacitinib or a placebo daily for 12 weeks, followed by upadacitinib for another 12 weeks. Ideal participants have experienced ongoing EoE symptoms despite using topical corticosteroids for at least eight weeks. As a Phase 2 trial, this research focuses on assessing the treatment's effectiveness in an initial, smaller group of people.
Will I have to stop taking my current medications?
You will need to continue taking your current topical corticosteroids and any other EoE treatments, like proton pump inhibitors or diet elimination, throughout the study. However, you cannot use systemic corticosteroids, dupilumab, estrogen, or certain other medications close to the start of the trial.
Is there any evidence suggesting that upadacitinib is likely to be safe for humans?
Research has shown that upadacitinib is well-tolerated by patients with conditions similar to eosinophilic esophagitis (EoE). Specifically, studies found that 91% of patients with severe and difficult-to-treat EoE experienced symptom improvement with upadacitinib, indicating that many patients found the treatment both manageable and effective.
However, collecting long-term safety information remains crucial, especially for younger patients. So far, no major safety issues have been consistently reported, but side effects can occur with any medication. While most participants in previous studies have seen improvements, individual experiences can differ.
Upadacitinib is already approved for other conditions, which provides some confidence about its safety. Since this trial is in an early stage, the researchers are still assessing its safety specifically for EoE. What evidence suggests that upadacitinib might be an effective treatment for Eosinophilic Esophagitis?
Research has shown that upadacitinib can help treat eosinophilic esophagitis (EoE). Are You a Good Fit for This Trial?
This trial is for adults aged 18-65 who have Eosinophilic Esophagitis (EoE) that isn't improving with swallowed steroid treatments. Participants must still have symptoms like trouble swallowing or chest pain, be willing to keep taking their current EoE medicines and diet, and be able to take pills.Inclusion Criteria

Treatment groups
Experimental Treatment
Participants randomly assigned to this arm will receive the active drug for the entire 24 week treatment period. During the first 12 weeks, participants will receive blinded upadacitinib 30mg daily (meaning the participant and study team will not know which medication the participant receives) followed by open-label upadacitinib 30mg daily for 12 weeks.
Participants randomly assigned to this arm will receive placebo for 12 weeks, followed by upadacitinib 30mg for 12 weeks. During the first 12 weeks, participants will receive blinded placebo daily (meaning the participant and study team will not know which medication the participant receives) followed by open-label upadacitinib 30mg daily for 12 weeks.
