L19IL2 + L19TNF for Basal Cell Carcinoma
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial explores a new treatment option for individuals with basal cell carcinoma, a common type of skin cancer, that has worsened despite other treatments. Researchers are testing the effectiveness of a combination of two agents, L19IL2 and L19TNF, injected directly into skin tumors to evaluate their efficacy and safety. This trial may suit those with locally advanced basal cell carcinoma, whose tumors can be injected and who have faced challenges with previous treatments. As a Phase 2 trial, the research focuses on assessing the treatment's effectiveness in an initial, smaller group, offering participants a chance to benefit from cutting-edge therapy.
Do I have to stop taking my current medications for the trial?
The trial does not specify if you need to stop taking your current medications. However, you cannot be on current topical or systemic chemotherapy or immunotherapy, and you should not be on chronic systemic steroid therapy exceeding 10 mg daily of prednisone equivalent.
Is there any evidence suggesting that this treatment is likely to be safe for humans?
Research has shown that treatments using L19IL2 and L19TNF have been tested for skin cancers like basal cell carcinoma. Early results suggest these treatments are generally well-tolerated. In one study, injecting L19IL2/L19TNF directly into melanoma tumors led to positive responses. However, about 48% of patients in other advanced cancer studies experienced serious side effects related to the treatment.
As these treatments are still in development, researchers are closely monitoring for potential issues. While some evidence suggests the treatments are safe, it remains incomplete. Previous studies with patients who have similar conditions indicate the treatment might be safe, but ongoing research will provide more information.12345Why do researchers think this study treatment might be promising for basal cell carcinoma?
Unlike the standard treatments for basal cell carcinoma, which often involve surgical excision, radiation therapy, or topical medications, L19IL2 + L19TNF offers a novel approach by using targeted immunotherapy. This treatment harnesses the power of cytokines, specifically interleukin-2 (IL2) and tumor necrosis factor (TNF), to directly stimulate the immune response against the tumor. Administered intratumorally, it delivers these potent immune-activating agents directly into the cancerous lesions, potentially increasing effectiveness and minimizing systemic side effects. Researchers are excited because this method could provide a less invasive and more precise alternative for patients with challenging or inoperable tumors.
What evidence suggests that L19IL2/TNF might be an effective treatment for basal cell carcinoma?
Research has shown that combining L19IL2 and L19TNF may help treat basal cell carcinoma (BCC). In earlier studies, this combination was tested on patients with non-melanoma skin cancers, including BCC, and demonstrated strong effects against tumors. Similar treatments often achieved success rates over 70%. L19IL2 enhances the immune system to attack cancer cells, while L19TNF directly kills tumor cells. Together, they aim to shrink tumors and halt their growth. These findings suggest that L19IL2/TNF could be effective for BCC patients who haven't responded well to other treatments.12346
Are You a Good Fit for This Trial?
This trial is for adults with locally advanced basal cell carcinoma (BCC) that has gotten worse or can't be treated with standard immune therapy. Participants must have at least one skin tumor that can be injected, good blood counts and organ function, and be able to provide a biopsy sample.Inclusion Criteria
Timeline for a Trial Participant
Screening
Participants are screened for eligibility to participate in the trial
Treatment
Participants receive intratumoral administrations of L19IL2/L19TNF into injectable cutaneous, subcutaneous tumors and nodal or in transit metastasis once weekly for up to 4 weeks. A second 4-week course may be administered for partial response or stable disease.
Follow-up
Participants are monitored for safety and effectiveness after treatment with tumor assessments at specified intervals.

Trial Overview
The study tests repeated injections of two drugs, L19IL2 and L19TNF, directly into BCC tumors once a week for up to 4 weeks. If the cancer responds or stays stable, another course may be given. All participants receive this treatment; there is no comparison group.
