Avacopan for Granulomatosis with Polyangiitis
What You Need to Know Before You Apply
What is the purpose of this trial?
This trial tests a medication called Avacopan to observe its behavior in the body over three days. It focuses on patients with a serious lung condition related to specific types of vasculitis (inflammation of blood vessels) who require mechanical breathing assistance. This trial may suit those diagnosed with severe granulomatosis with polyangiitis (GPA) or microscopic polyangiitis (MPA) experiencing serious lung bleeding. As a Phase 1 trial, the research aims to understand how the treatment works in people, offering participants the opportunity to be among the first to receive this new treatment.
Do I have to stop taking my current medications for the trial?
The trial does not specify if you need to stop taking your current medications, but you cannot use a strong CYP3A4 inhibitor while participating.
Is there any evidence suggesting that avacopan is likely to be safe for humans?
Research has shown that avacopan is generally well-tolerated. Patients with conditions like GPA (granulomatosis with polyangiitis) and MPA (microscopic polyangiitis) have used avacopan with good results. In one study, only 14% of patients experienced serious infections over six months, indicating a relatively low rate of serious side effects. Another study found that people taking avacopan had fewer unwanted effects and infections compared to those not taking it.
Avacopan is already approved in the U.S. for treating severe active GPA and MPA in adults, meeting safety standards for these conditions. This suggests it is generally safe, although individual experiences can differ. Prospective trial participants should discuss their specific health situation with the trial team.12345Why do researchers think this study treatment might be promising?
Unlike the standard of care for Granulomatosis with Polyangiitis, which typically involves corticosteroids and immunosuppressants, Avacopan offers a novel approach by targeting the complement 5a receptor. This mechanism allows it to specifically modulate the immune response without the broad immunosuppressive effects of traditional treatments. Researchers are excited about Avacopan because it could potentially reduce the need for high-dose steroids, which are known for their significant side effects. Additionally, Avacopan's administration via an NG tube in a solubilized form is unique, potentially allowing for more precise dosing and quicker onset of action.
What evidence suggests that avacopan might be an effective treatment for GPA?
Studies have shown that avacopan effectively treats conditions like granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). Specifically, research indicates that avacopan led to disease remission in about 74% of patients at week 26, compared to 71% with standard treatment. Patients using avacopan also maintained remission, with about 68% still in remission at week 52. This medication reduces the need for other drugs like glucocorticoids, which often have side effects. Overall, avacopan has proven both effective and safe in managing these conditions. Participants in this trial will receive avacopan as the investigational treatment.12367

Are You a Good Fit for This Trial?
This trial is for adults with severe, active granulomatosis with polyangiitis (GPA) or microscopic polyangiitis (MPA), who have lung bleeding and need a breathing machine. Participants must test positive for certain antibodies and require standard treatment. People with liver problems, certain infections, pregnancy, or allergies to avacopan cannot join.Inclusion Criteria
